The Wholeistic Wellness Self-Assessment is a comprehensive mind-body-spirit evaluation for greater health, balance, and purpose, containing 18 areas of wellness.

Wellness is a broad concept that incorporates every aspect of your life, from what you eat to what you scroll to your mental and emotional states. Wellness is about finding balance in life, understanding your needs, and taking intentional steps to nurture and heal yourself.

We should all care about wellness because it directly impacts every aspect of our lives – health, happiness, relationships, jobs, and longevity. Wellness is not a luxury – it is the FOUNDATION for living a fulfilling, loving, authentic, balanced, and resilient life.

This assessment is purely for your own benefit, though I encourage you to share your insights with loved ones who are supporting your wellness journey. My other hope, of course, is that you will identify areas where your wellness routines/habits can use improvement/growth.

Please remember: There are no right or wrong answers to this assessment; it is meant purely for your benefit in understanding the many facets of wellness, which is why there is also no scoring system. The intent here is to help you see where you are thriving with wellness and where you might want to put in more time and work.

Wellness Self-Assessment

Directions: Start by reading each prompt, then honestly answering each question. Focus on leaning into these prompts and examining your life with curiosity and an eye toward improving your wellness habits.

  1. Nutrition: Are you eating a diet of mostly real foods that nourish your body and brain?

  2. Movement: Are you making movement and exercise a daily (even hourly) priority?

  3. Nature: How much time, on average, do you spend outside in nature – and is it enough?

  4. Healing: Are you actively trying to heal from difficult experiences in your past and learning to move forward with greater self-love and self-compassion?

  5. Mental: Do your thoughts generally support your well-being, growth, and ability to navigate life's challenges?

  6. Stress/Life: How much chronic stress do you feel, and how is the pace of life?

  7. Sleep: Are you achieving consistent, regular, and quality sleep?

  8. Purpose: Do you have a strong sense of direction, meaning, and purpose for your life?

  9. Spirituality: Do you feel connected with a higher power? Do you practice gratitude?

  10. Emotional: Do you express and manage your emotions in healthy ways?

  11. Intellectual: How much time do you spend supporting your brain?

  12. Creative: Do you have outlets for your creativity – hobbies, journaling, creating, etc.?

  13. Digital: Do you control your technology use – including social media, videos, and AI – or does it often distract you from what matters most?

  14. Relationships & Community: How do you feel about the number and quality of the people in your life? Do you have mostly healthy and supportive relationships?

  15. Financial: Do you feel in control of your finances, spending habits, and financial future?

  16. Substance: Are you avoiding unhealthy substances, such as smoking, alcohol, or drugs?

  17. Self-Care: Do you actively make time for activities that recharge you mentally, physically, or spiritually?

  18. Environmental: Does your living and working environment support your health and well-being?

Next Steps: Review your responses to each area of wellness and identify:

  • Your top 3 wellness strengths – the areas where you are excelling.

  • Your top 3 wellness growth areas – the areas that you want to improve.

  • One action step in the next 30 days to build a habit to improve one of those wellness growth areas.

Attaining wellness is a lifelong pursuit, not something easily accomplished in a short period – and this should be encouraging to you.

If the assessment results opened your eyes to areas where you need to focus, that gives you direction.
